If you want to allow your child to walk home from school, we must have this form signed and returned
to the school. Your child will not be allowed to leave campus unaccompanied unless and until this form
has been received by the school. THIS FORM ONLY APPLIES TO THOSE STUDENTS WHO WILL WALK
HOME. YOU SHOULD ALREADY HAVE ON FILE WITH THE SCHOOL THE PERSON(S) DESIGNATED TO PICK
UP YOUR CHILD (FILLED OUT DURING ENROLLMENT).
My child has permission to WALK HOME (walk off campus) from Uplift Summit. I am aware that once
my child leaves the school campus, there is no adult supervision for them. I have made my child aware
that, for their safety, they are not allowed to loiter around the premises after school or at any nearby
businesses, and they are to go directly to their after school destination.

You are aware of student dismissal time: 3:30 pm (Monday, Tuesday, Thursday, Friday); 2:00
pm (Wednesday).
You will assist Uplift Summit in ensuring your child’s safety by adhering to the guidelines
outlined above.
You agree that ANY CHANGES from the above permission will be communicated (in writing) to
the front office. This permission form will remain in effect unless and until you have notified the
school in writing that it has been rescinded.
You agree to the sibling pick-up procedures attached.

forma y la regrese a la escuela. No se permitirá que su alumno abandone la escuela sin que algún adulto
lo recoja si esta forma no ha sido recibida por la escuela. ESTA FORMA SOLO APLICA AQUELLOS
ALUMNOS QUE SE IRAN A CASA CAMINANDO. USTED YA DEBE TENER EN SU PERFIL LAS PERSONA
DESIGNADAS Y AUTORIZADAS PARA RECOGER A SU ALUMNO. (ESTO LO COMPLETO DURANTE LA

Mi alumno tiene permiso de IRSE CAMINANDO A CASA (abandonar el campus) de la PRIMARIA UPLIFT
SUMMIT. Estoy consciente que una vez que mi alumno abandona el campus, no habrá ningún adulto
para supervisarlo. Se lo he explicado a mi alumno. Por su seguridad, NO se les permitirá quedarse en la
escuela o en los negocios cercanos, deben de irse directamente a su destino después de clases. Si mi
alumno esta en UPLIFT SUMMIT después de las 4:15 pm.

Esta consiente de que su alumno sale de la escuela a las 3 30pm lunes, martes, jueves y viernes
Apoyará a Uplift en asegurarse que su alumno está seguro siguiendo las reglas antes señaladas.
Está de acuerdo que si decide cambiar de opción lo comunicara a la escuela de inmediato y por
escrito a la Oficina Principal. Esta forma se mantendrá en efecto hasta que usted nos notifique
por escrito de lo contrario.

Walk/Drive Sibling Pick-up

Families must fill-out the walk home or drive home form and submit to the office.
Primary scholars will only be approved to walk home if they are accompanied by a
sibling.
Scholars that walk or drive home will be issued an ID that indicates that they have an
approved walk/drive home form submitted to the school.
o Scholars that turn-in their form after the IDs have been printed, will need to pay
$5 to get a new ID with the walk/driver designation.
Middle school scholars that are approved to walk home, and have a primary sibling, will
be escorted over to the primary campus far door between 3:25-3:30. Scholars must
have their sibling’s carline number to pick up their sibling.
High school scholars that are approved to walk/drive home, and have a primary sibling,
will pick up their sibling up from the primary school main door between 4:004:10. Scholars must have their sibling’s carline number to pick up their sibling.
High school scholars that are approved to walk/drive home, and have a middle school
sibling, will pick their sibling up from middle school building by 4:15.
Only scholars that have walker/driver on their ID badge will be allowed to cross over to
the primary campus.
After scholars have collected their siblings, they must immediately leave campus to
walk or drive home.

Las familias deberán de llenar una forma en la oficina
Los alumnos de Primaria se las autorizara caminar a casa SOLAMENTE si son acompañados por
sus hermanos
Los alumnos que caminan o manejan a casa se les otorgara una identificación que tienen
permiso registrado en la escuela para hacerlo.
Los alumnos que entreguen la forma después que se hayan distribuido las identificaciones se les
cobrara $5 para obtener una nueva
Los alumnos de la Secundaria que tienen permiso para recoger a sus hermanos de la Primaria y
caminar a casa serán acompañados al edificio de la Primaria entre las 3 25pm y 3 30pm. Los
alumnos deben tener el número de su hermano/a para poder recogerlo/a.
Los alumnos de la Preparatoria que tienen permiso para recoger a sus hermanos de la Primaria y
caminar a casa/manejar deben de ir al edificio de la Primaria por la puerta principal entre las
4:00pm y 4:10pm. Los alumnos deben tener el número de su hermano/a para poder
recogerlo/a.
Los alumnos de la Preparatoria que tienen permiso para recoger a sus hermanos de la
Secundaria y caminar a casa o manejar podrán hacerlo en el edificio de la Secundaria a más
tardar a las 4:15pm.
Solamente los alumnos que tienen la identificación con la distinción de caminar/manejar se les
otorgara permiso de cruzar a la Primaria.
Después, de que los alumnos hayan recogido a sus hermanos, deberán abandonar la escuela
inmediatamente ya sea caminando o por auto.
