REQUIREMENTS
If your current Florida DL or ID card
reflects a gold star in the top right corner
and NO changes are now being
requested, proof of identity
documentation will not be required.
Proof of Identity
Florida law and federal guidelines require ALL applicants for a new,
new, renewal,
renewal, or replacement driver
license or identification card that are not currently federal ID compliant, to present the following
identification documents:
• An original or certified copy of a US birth certificate, valid U.S. Passport, valid I-551 / I-151
Permanent Resident Card (green card) or original Certificate of Naturalization (no copies or hospital
certificates will be accepted);
AND
• A Florida or out-of-state driver license or identification card, or an acceptable secondary form of
identification;
AND
• An original Social Security card (no photocopies) or acceptable proof of Social Security number (see
complete list on reverse).
AND
• Two proofs of Florida residential address from two different sources, with the exception of mail from
the Florida Division of Driver License (see complete list on reverse);
AND
• Original, court order, or government issued certified copy of all name change documents (marriage,
adoption, legal name change, etc.) must be presented at the time of application (may require more
than one);
Social Security card and identity records must match the name to be placed on a Florida driver license
or identification card. Contact the Social Security Administration at least 24 hrs before making
application for a driver license or identification card when changing names using a corrected W-2 or
federal form 1099 as proof of Social Security number.
NOTE: Applicants under age 18 must have signed parental consent.

Proof of Residential Address Includes Two of the Following:
1.
Deed, mortgage, monthly mortgage statement, mortgage payment booklet, or residential
rental/lease agreement.
2.
Florida voter registration card.
3.
Florida vehicle registration or title.
4.
Florida vessel registration or title (if living on a vessel / houseboat).
5.
Statements:
• Customers under 18 - a statement from a parent, stepparent, or legal guardian of the minor is
required. The parent, guardian, or stepparent must reside at the same residential address as the
minor. The parent, guardian, or stepparent who accompanies the minor will give a verbal statement
that the minor resides at the same address and present two proofs of residential address. The
parent, guardian, or stepparent who cannot accompany the minor must complete a statement
through a Certification of Address form with two proofs of residential address.
• Customers 18 years or older – customers who reside with someone and receive no mail in their own
name must provide a statement through a Certification of Address form from that person, along with
two proofs of residential address in that person’s name.

A utility hook-up or work order dated within 60 days of the application.
7.
Automobile payment booklet.
8.
Selective Service card.
9.
Medical or health card with address listed.
10.
Current homeowner’s insurance policy or bill.
11.
Current automobile insurance policy or bill.
12.
Educational institution transcript forms for the current school year.
13.
Un-expired professional license issued by a government agency in the U.S.
14.
W-2 form or 1099 form.
15.
Form DS2019, Certificate of Eligibility for Exchange Visitor (J-1) status.
16.
A letter from a homeless shelter, transitional service provider, or a halfway house verifying that
the customer resides at the shelter address.
17.
Utility bills, not more than two months old.
18.
Mail from financial institutions, including checking, savings, or investment account statements,
not more than two months old.
19.
Mail from Federal, State, County, or City government agencies, with the exception of mail from
the Florida Division of Driver License.
20.
Transients – sexual offender/predator/career offender: FDLE Registration form completed by local
sheriff’s office.
Proof of Social Security Number
Social Security number is necessary pursuant to Chapter 322, Florida Statutes.
Acceptable Documents Are:
• Social Security card
• W-2 form
• SSA form 1099
• Any other valid form 1099
• Paycheck stub showing your full name and Social Security number

4. La tarjeta de seguro social y documentos de identidad deben coincidir con el nombre que usted

Si usted necesita hacer un cambio de nombre en su licencia, debe ir a la oficina de seguro social para
pedir el cambio. Esperar por lo menos 24 horas antes de visitar la oficina de licencias.
5. Menores de 18 años. En caso de que los padres o tutores no puedan estar presentes cuando el
solicitante visite la oficina de licencias, necesitara tener consigo una carta firmada por los padres o
tutores y notariada, otorgándole permiso al menor de edad. Pueden encontrar la forma de

menor darán una declaración verbal de que el menor vive en la misma dirección y presentaran 2
pruebas de dirección de 2 diferentes fuentes y la forma certificación de dirección firmada por los padres

carta verificando que usted vive en dicho lugar. Se necesita la dirección física.
