CONDICIONES DE SUPERVISION
CONDITIONS OF SUPERVISION
Instrucciones: Favor de marcar con una “X” las condiciones que el ofensor deberá cumplir mientras sea supervisado en
Puerto Rico. El cliente deberá colocar sus iniciales en el espacio provisto al lado de cada condición que se le imponga
como evidencia de que fue orientado. Además, tanto el cliente como el Técnico de Servicios Sociopenales deberán firmar
en el espacio provisto al final de la hoja.
Instructions: Please mark with an “X” the conditions the defendant shall abide while supervise in Puerto Rico. The defendant shall write his/her initials in the provided space next to each
imposed condition as evidence that he/she was oriented. Besides, the defendant as well as the PO shall sign in the space provided at the end of this document.
CONDICIONES REGULARES DE SUPERVISION
REGULAR CONDITIONS OF SUPERVISION
1. Deberá buscar, obtener y mantener empleo o estudios. Someterá evidencia al Técnico de Servicios
Sociopenales asignado.
Shall seek, obtain and maintain employment or studies. Submit evidence to P.O.
2. Deberá someterse a pruebas al azar de detección de sustancias controladas, evaluación de abuso de
drogas/alcohol, y completar tratamiento, de ser necesario. El/la ofensor/a costeará los gastos, de
ser necesario.
Shall submit to random drug tests, drug/alcohol abuse evaluation and complete treatment at the defendant’s expense, if necessary.
3. Si el/la ofensor/a resulta positivo al uso de sustancias controladas, el/ella recibirá tratamiento
interno, de ser necesario. El/la ofensor/a costeará los gastos, de ser necesario.
If the defendant results positive to the use of controlled substances, he/she shall receive in-patient treatment at the defendant’s expense, if necessary.
4. El rechazo del ofensor/a a someterse a una prueba de detección de sustancias controladas se
considerará como un resultado positivo administrativo.
The refusal of the defendant to submit to a drug test will be considered as an administrative positive result.
5. Deberá abstenerse del uso de sustancias controladas y/o alcohol.
Shall abstain from the use of controlled substances and/or alcohol.
6. Deberá someterse a evaluación médica, sicológica y/o siquiátrica y completar tratamiento, de ser
necesario. El/la ofensor/a costeará los gastos, de ser necesario.
Shall submit to medical, psychological and/or psychiatric evaluation and complete treatment at the defendant’s expense, if necessary. Maintain the PO
informed of its progress.
7. Deberá abstenerse de conducir bajo la influencia de alcohol/sustancias controladas.
Shall abstain from driving under the influence of alcohol/controlled substances.
8. Deberá participar de reuniones de A.A. (Alcohólicos Anónimos) y/o N.A. (Narcóticos Anónimos),
de ser necesario. El/la ofensor/a costeará los gastos, de ser necesario.
Shall participate of A.A. (Anonymous Alcoholics) and/or N.A. (Anonymous Narcotics) meetings at the defendant’s expense, if necessary.
9. Deberá completar curso de anti-agresión/anti-violencia (manejo del coraje), de ser necesario. El/la
ofensor/a costeará los gastos, de ser necesario.
Shall complete anti-aggression/anti-violence counseling (anger management) at the defendant’s expense, if necessary.
X 10.Deberá cumplir restricción domiciliaria: estar diariamente en su residencia de 9:00 pm a 6:00 am,
excepto si está estudiando, trabajando o participando de alguna actividad religiosa.
Shall observe curfew: be everyday in his residence no later than 7:00 pm until 6:00 am, unless studying, working or attending religious activities.
11. Deberá obtener autorización escrita de su Técnico de Servicios Sociopenales asignado previo a
cualquier cambio temporero o permanente de residencia y/o salida de la jurisdicción.
Shall obtain the written approval of the assigned P.O. prior to any temporary or permanent change of residence and/or leaving the jurisdiction
.
12. No deberá viajar a ninguna jurisdicción que no forme parte del Pacto Interestatal para la
Supervisión de Ofensores Adultos (Viajes Internacionales).
Shall not travel to any jurisdiction which is not part of the Interstate Compact for Adult Offenders Supervision. (International Travel)

13. Deberá someterse a evaluación de abuso de alcohol y completar tratamiento, de ser necesario.
El/la ofensor/a costeará los gastos, de ser necesario.
Shall submit to alcohol abuse evaluation and complete treatment at the defendant’s expense, if necessary.
14. No deberá frecuentar bares o establecimientos donde se expendan bebidas alcohólicas/sustancias
controladas como negocio principal.
Shall not frequent bars or establishments were alcohol/drug is sold or served as its main business.
15. No deberá asociarse con personas relacionadas al tráfico de drogas y/o de dudosa reputación.
Shall not associate with people related to drug trafficking and/or people of doubtful reputation.
16. Deberá saldar toda deuda que tenga con el estado de origen, preferiblemente antes de la llegada a
nuestra jurisdicción.
Shall settle any monetary balance with the sending state, preferably prior to the arrival to our jurisdiction.
17. No deberá intervenir con la víctima y/o sus familiares.
Shall not intervene with the victim or the family.
18. El estado de origen deberá retomar al ofensor/a si este/a falla en cumplir con las condiciones
impuestas en el estado receptor.
The sending state shall retake the offender if the defendant fails to comply with the imposed conditions in the receiving state.
19. Deberá regresar al estado de origen si el familiar residente propuesto retira su disposición a asistir
al ofensor/a o si el/la ofensor/a falla en cumplir con las condiciones impuestas en el estado
receptor.
Shall return to the sending state if the proposed resident family withdraws its willingness to assist the defendant or if the defendant fails to comply with
the imposed conditions in the receiving state.
20. Deberá obedecer todas las leyes, ordenanzas, códigos y regulaciones de Puerto Rico y las
instrucciones que provea el Técnico de Servicios Sociopenales.
Shall obey all PR’s laws, ordinances, codes, regulations and the instructions provided by P.O.
CONDICIONES DE SUPERVISIÓN PARA OFENSORES SEXUALES
CONDITIONS OF SUPERVISION FOR SEX OFFENDERS
1.
Deberá reportarse en el Cuartel de la Policía del Aeropuerto inmediatamente llegue a Puerto Rico
y completará los formularios de ofensores sexuales.
Shall report to the Police Station of the Airport immediately upon his arrival to Puerto Rico and complete the sex offender’s forms.
2.
Deberá registrarse en el Registro de Ofensores Sexuales y Abuso a Menores dentro de los
siguientes diez (10) días de su llegada a Puerto Rico según dispone la Ley Núm. 266 del 9 de
septiembre de 2004. El/la ofensor/a deberá permanecer en el Registro por un período mínimo de
diez (10) años luego de finalizada la sentencia impuesta. Deberá cumplir con el registro anual.
Shall register in the Sex Offender and Child Abuse Registry within ten (10) days of the arrival pursuant to Law #266 of September 9, 2004. The offender
shall remain registered for at least ten (10) years after the sentence’s maximum expiration date. Shall comply with the annual registration.

Deberá obtener aprobación escrita de su Técnico de Servicios Sociopenales previo a cualquier
cambio temporero o permanente en residencia y/o salida de la jurisdicción para cualquier asunto,
incluyendo visita familiar, cita médica, y/o actividad religiosa. El cambio de dirección deberá
registrarse en el Registro de Ofensores Sexuales y Abuso a Menores con diez (10) días de
anticipación según dispone la Ley Núm. 266 del 9 de septiembre de 2004. Deberá presentar toda
la evidencia que sea necesaria o solicitada por el Técnico de Servicios Sociopenales.
Shall obtain written approval of the assigned P.O. prior to any temporary or permanent change of residence and/or leaving the jurisdiction for any matter,
including family visits, medical appointments, and/or religious activities. The change of address shall be registered in the Sex Offender and Child Abuse
Registry within ten (10) days in advance pursuant to Law #266 of September 9, 2004. Shall present any evidence deemed necessary or requested by P.O.

Shall submit to sex offender’s evaluation and complete treatment at the defendant’s expense, if necessary.
5.
No deberá tener contacto sin supervisión con menores de edad. “Contacto” significa encontrarse
en persona, por teléfono, por correspondencia o indirectamente a través de terceras personas.
Cualquier contacto supervisado con menores deberá aprobarse por el Técnico de Servicios
Sociopenales. “Contacto Supervisado” significa que cualquier contacto con menores tiene que ser
físicamente monitoreado en todo momento por un adulto designado. “Físicamente Monitoreado”
significa estar presente y observar visualmente todo contacto. “Adulto Designado” significa un
adulto responsable que ha sido aprobado por el Técnico de Servicios Sociopenales.

Contacto supervisado no incluye visitas o estadías para quedarse a dormir. Esto significa que
usted no estará solo/a con ningún menor de edad.
Shall not have unsupervised contact with minors. “Contact” means face-to-face, telephonic, correspondence, or any indirect contact via third parties.
Any supervised contact with minors must be specifically approved by the PO. “Supervised Contact” means that any contact with minors has to be
physically monitored by a designated adult at all times. “Physically Monitored” means being present to visually observe all contact. “Designated Adult”
means a responsible adult who has been approved by the officer. Supervised contact does not include overnight visits or lodging. This means you will
not be alone with any minor.
6.
No deberá vivir, visitar o estar a menos de 500 pies de distancia de parques, escuelas, centros de
cuido de niños, piscinas, playas, cines, u otros lugares donde se congreguen menores sin la
autorización anticipada de su Técnico de Servicio Sociopenal. En caso de que ocurra algún
contacto incidental con un menor deberá reportarlo al Técnico de Servicios Sociopenales dentro
de veinticuatro (24) horas.
Shall not reside, visit, or be within 500 feet from parks, schools, day care centers, swimming pools, beaches, theaters, or other places where children
congregate without advance approval of your PO. Incidental contact with children shall be reported to the PO within twenty-four (24) hours.
7.
No deberá poseer equipo de acceso a Internet o acceso a Internet.
Shall not possess internet access equipment, or access the internet.
8. No deberá poseer ningún material explícitamente sexual o sexualmente violento.
Shall not possess any sexually explicit or sexually violent materials.
9.
No deberá deambular, pasear, caminar y/o correr por el vecindario sin la supervisión de un Adulto
Designado previamente aprobado por el Técnico de Servicios Sociopenales.
Shall not stroll, circulate, walk and/or run in the neighborhood without the supervision of a Designated Adult approved by the P.O.
