Participation Waiver
I am aware that participation in Fort Worth All Stars League or use of soccer field equipment creates risk of injury, and I,
on behalf of myself and participant’s knowingly and freely assume all such risks, both know and unknown, even if arising
from negligence of other; and,
I for myself and the participant(s) listed above and our respective heirs, assigns, administrators, personal
representatives, and next of kin, hereby release and hold harmless, Fort Worth All Stars League and their affiliates,
officers, members, volunteers, agents, employees, other participants, and sponsoring agencies from and against any and
all claims, injuries, liabilities or damages arising out of or related to participation any way and all programs of All Stars
soccer league and/or the use of any field equipment.

Exclusión de Responsabilidad
Soy consciente de posibles riesgos de accidente, al participar en Fort Worth All Stars League y en el uso de las canchas y
equipo de futbol, en mi nombre y del participante sabiendo y asumiendo libremente todos los riesgos conocidos y que
puedo no conocer aun creados por negligencias de otros; y,
Yo por mi propia voluntad y del participante nombrado arriba y nuestros herederos, cesionarios, administradores,
representadores personales, parientes más cercanos por la presente renuncio y mantendré sin daño alguno a Fort
Worth All Stars League y sus afiliados, oficiales, miembros, voluntarios, agentes, empleados, otros participantes, y
agencias patrocinadoras por todos y cada uno de los reclamaciones, lesiones, daños derivados relacionados con la
participación de cualquier manera en el programa de Fort Worth All Stars League y/o el uso de cualquier campo de
juego.
