El Formulario de la Otorga de Derechos Limitados debe ser
usado en un caso donde el guardián legal de un niño desea que
otro adulto (quien no es un guardián legal) participe en las
decisiones educacionales tomados referente a ese niño. En el
caso de padres divorciados, no es necesario llenar este
formulario para dar derechos al consevador posésivo (el padre
que no tiene cusodia del niño la mayor parte del tiempo) si el
decreto del divorcio ya le da esos derechos.
Firmando este formulario y consiguiendo el notarizacieon, el
guardián legal otorga el derecho a un adulto que no sea
guardián de actuar en lugar del guardián en las siguientes
circunstancias:
1) El adulto no-guardián puede firmar una nota dando permiso
para que el niño viaja en un viaje de campo o evento
patrocinado por la escuela.
2) El adulto no guardián puede corresponder (por escrito, por
teléfono, por correo electrónico, o en persona) con personal
de la escuela sobre las calificaciones, asistencia, disciplina,
o colocación educativo del niño. Este formulario no garantiza
el derecho del no-guardián de tomar decisions en estas
areas, so esta el intento de facilitár el proceso de
comunicación entre la escuela y el hogar. El guardián legal
mantiene su derecho legal de ser el que toma decisiones
para el estudiante.
3) El no-guardián puede sacar el niño de la escuela en un caso
donde el guardián no está disponible para hacerlo. Este
occure de vez en cuando cuando familias se mueven a un
lugar distante y el guardián se ha permanecido atrás para
completar este face de la mudanza, el o ella puede firmar los
papeles de retiro en lugar del guardián. En este caso, el
guardián debiera contactar la escuela en anticipo para dar
conocer al personal de la escuela que este va a suceder.
Estos son los únicos derechos afectados por este formulario. Si
el formulario no estea notorizado, la escuela no puede aceptarlo.
Si en cualguier momento el guardián legal desea revocar estos
derechos, el o ella debe hacerlo por escrito.
Este formulario se usaría la mayor parte del tiempo en el caso
que guardiánes legales otorgan dichos derechos a padrastros,
pero no se limite a ese situación. No es necsario tener este
formulario archivado para designar personas de contacto de
emergencia o personas que puedan recojer el estudiante de la
escuela. Padres pueden designar estos individuos, por escrito,
en la tarjeta de emergencia y documentos de la clínica llenados
durante el proceso de registro. El Formulario de Otorga de
Derechos Limitados otorga derechos que no están cubiertos por
la tarjeta de emergencia y documentos de la clínica.
The Granting of Limited Rights Form is to be used in a case in
which the legal guardian of a child wishes to allow another adult
(who is not a legal guardian) to be involved in the educational
decisions made about that child. In the case of divorced parents,
it is not necessary to fill out this from to give rights to the
possessory conservator if the divorce decree already grants
those rights.
By signing this form and having it notarized, the legal guardian
gives the right to a non-guardian adult to act on the guardian’s
behalf in the following circumstances:
1) The non-guardian may sign a note giving permission for the
child to travel on a school-sponsored field trip or event.
2) The non-guardian may correspond (in writing, by telephone
or email, or in person) with school personnel regarding the
student’s grades, attendance, discipline, and educational
placement of the child. This form does not guarantee the
right of the non-guardian to make decisions in these areas; it
is merely intended to facilitate the communication process
between the school and home. The legal guardian maintains
his/her legal right to be the decsion-maker for the student.
3) The non-guardian may withdraw the child from school in the
case in which the guardian is unavailable to do so. This
happens at times when families are moving to a district
location and the guardian has already left the Conroe ISD
area. If the non-guardian has remained behind to finish this
end of the move, he/she may sign the withdrawal papers in
lieu of the guardian. In this case, the guardian should contact
the school in advance to let the school’s personnel know that
this will happen.
These are the only rights address by this form. If the form is not
notarized, the school cannot accept it. If, at any time, the legal
guardian wishes to revoke these rights, he/she must do so in
writing.
This form would most often be used in the case of legal
guardians granting such rights to step-parents, but is not limited
to that situation. It is not necessary to have this form on file to
designate emergency contacts or people who may pick up the
student from school. Parents may designate these individuals, in
writing, on the emergency card and clinic documents filled out
during the registration process. The Granting of Limited Rights
Form grants rights that are not covered by the emergency card
and clinic documents.
