Vashon Island School District No. 402 Loan of computer for use at home Student/Parent Contract Dear Families, Vashon Island School District is replacing some of our computers, and wish to loan those computers to students to use at home to complete their school work. Your student is eligible. If you are interested, please circle your response to the following 3 questions, then sign the agreement below. 1. Do you have internet access at home? (If you have no internet access, then you are not eligible for a computer at this time.) YES NO 2. Do you have a working computer at home? YES NO 3. Do your middle and high school aged children have sufficient access to that computer for their school work? YES NO I accept the loan of a computer from the Vashon Island School District for the 2015­16 school year and assure that my child and I are responsible for the care and maintenance of this machine. If the computer is lost, broken, or stolen, my child will volunteer 50 hours in their school community to honor the trust of this loan.
