Media Release Form
Anaheim Elementary School District is proud to promote the success of its students, staff and programs. These accomplishments may draw the
attention of newspapers, television stations and other media who visit our schools to photograph, film and identify students and staff during various
activities.
The District itself also uses names, images and video clips of students in materials such as newsletters, website content and postings to official
social media platforms.
To grant the District and those acting under its permission the ability to feature your student, please complete this Media Release Form. If
the District does not receive this form, your student will NOT BE included in materials for public promotion.
A separate signature form is required for each child in school. Please sign below and return this form to your child’s school.

El Distrito de Escuelas Primarias de Anaheim se enorgullece de promover el éxito de sus estudiantes, su personal y sus programas educativos. Estos
logros pueden atraer la atención de los periódicos, los canales de televisión y otros medios de comunicación que visitan nuestras escuelas para fotografiar,
grabar e identificar a nuestros estudiantes y personal durante distintas actividades.
Además, el mismo Distrito utiliza los nombres, las imágenes o grabaciones de los estudiantes en materiales como los boletines, el contenido de la página
de internet y publicaciones en las plataformas oficiales en los medios sociales.
Para otorgarle al Distrito y a aquellos quienes actúan con su permiso, la autorización de destacar a su estudiante; por favor, llene este
FORMULARIO PARA LA DIVULGACIÓN PUBLICITARIA. Si el Distrito no recibe este formulario, NO incluiremos a su estudiante en los materiales de publicidad.
Es necesario un formulario firmado para cada estudiante en la escuela. Por favor, firme a continuación y regrese este documento a la escuela de
su hijo/a.

I hereby grant to Anaheim Elementary School District, its successors, and those acting under its permission and on its authority the right and
permission to copyright, use, publish, display, produce, duplicate, and distribute photographic, video, or sound recording reproductions, portraits, or
pictures of my child made through any media at its studios or elsewhere.
I hereby release, discharge and agree to hold harmless the Anaheim Elementary School District, its members, successors and those acting under its
permission and on its authority from any liability to the extent permitted by law, for the preparation, processing or distribution of the production.
I, the parent and/or legal guardian of ___________________________________________________, do consent and grant my permission to all of
the above.

Por la presente, otorgo al Distrito de Escuelas Primarias de Anaheim, a sus sucesores, y a todos aquellos que actúen bajo su permiso y con su autoridad el
derecho y el permiso de solicitar el derecho de autor, utilizar, publicar, mostrar, producir, duplicar, y distribuir las reproducciones de fotografías y/o de
grabaciones de videos o audios, retratos, o imágenes de mi hijo/a a través de cualquier medio, en sus estudios u otro lugar.
Por la presente, libero y exonero al Distrito de Escuelas Primarias de Anaheim, a sus miembros, a sus sucesores, y a todos aquellos que actúen bajo su
permiso y en su autoridad de toda responsabilidad civil en la medida que lo permita la ley para la preparación, procesamiento, o distribución de la
producción.
