Dear SoonerCare Member,
Welcome to the SoonerCare Disease Management Program! You have been selected for this free service. This
new program is a free part of your SoonerCare coverage. This program will help members like you to improve
your health.
Trained staff from the program will be calling you several times a year to check on your health, give you helpful
tips, answer your questions, and help you manage your health. Our goal is for you to live a longer healthier life.
You will get your first phone call from our staff in a few weeks.
During the first call, we will answer questions you have about the new program. The staff will ask you some
questions about your health, medicines, medical supplies, and visits to doctors and hospitals. This will help us
provide you with information about your sugar diabetes. This first call will only take a few minutes and will help
us know how to help you. If we do not reach you, we will mail you the information and ask that you call us at
405-522-6205 or the toll-free number 800-522-0114. After the first call, you will receive a mailing about diabetes.
We look forward to talking with you about ways that we can help you manage your diabetes and overall health.
