Students who do not return a signed permission form will be placed in a study skills room for the day. Students who are not dressed appropriately (in business attire) will be placed in a study skills room for the day. This permission form has been signed only after understanding and considering the following information: 1. Trip Planned : Juniors will tour the Lanier Tech/UNG Campus in Oakwood in the morning. That afternoon, each advisement group will job shadow at a variety of businesses in Gainesville, including, but not limited to: The Gainesville Times, Probate Court, Syfan Logistics, ZF, IMS Gear, Hall County Fire Services, Kipper Tool, MP Equipment, Baldor, Chick‐fil‐a Gainesville, Belk, SKF, NEGA Medical Center, and Kings Hawaiian. 2. Purpose of Trip : Educational information about college opportunities and career experiences after graduation. Supervision : Teachers will be with students throughout the day approximately 1 teacher per 18 students 3. Transportation : Hall County school bus 4. Dress Code: Specific dress code instructions will be given by your advisement teacher. YOU MUST ADHERE TO THE DRESS CODE. 5. Insurance : I understand that the Board of Education does not or may not carry any insurance relative to the trip for injuries to the student. My child has insurance through the Board’s student insurance program or through my own insurance carrier. 6. Funds Requested : Amount: None (Lunch will be provided) Contribution of the above requested funds is purely voluntary. Your child will in no way be denied participation or penalized if you do not contribute. I request that the above named student be allowed to participate in the trip planned and specifically consent to his/her participation.I also authorize the school to obtain through a physician of its own choice any emergency care that may become reasonably necessary for the student during the trip. Esta forma de permiso ha sido firmada solo después de haber entendido y considerado la siguiente información: 1. Mi hijo/a tiene seguro a través del programa de la Junta de Seguros para el Estudiante o a través de mi propio seguro. 6. Fondos Requeridos: Cantidad: Ninguna, sin costo (se proveerá el almuerzo) La contribución de los fondos requeridos para el estudiante es únicamente voluntaria. Su hijo/a no será rechazado/a en ninguna forma o penalizado/a si usted no coopera. Yo requiero que el nombre del estudiante arriba mencionado se le permita participar en el viaje planeado y específicamente doy mi consentimiento para su participación. Yo también autorizo a la escuela para obtener por medio de un doctor de su propia elección, cualquier atención de emergencia que pudiera presentarse, razonablemente necesaria, durante el paseo.
