INTRODUCCION: Staphylococcus lugdunensis es una especie de estafilococo coagulasa
negativo, inicialmente relacionado con endocarditis, posteriormente, se ha asociado a un amplio
espectro de infecciones en su mayoría leves, pero más habituales que la endocarditis, siendo las
más frecuentemente descritas las infecciones de la piel y de tejidos blandos. Teniendo como
objetivo, identificar S. lugdunensis en aislados y establecer su relación patógena asociada a
procesos infecciosos, y establecer un sistema de detección, que sea factible de realizar por los
laboratorios de microbiología del país.
MATERIAL Y METODOS: Se presenta un estudio descriptivo, observacional de corte
transversal. Se estudiaron los aislamientos de S. lugdunensis del Hospital Nacional Daniel
Alcides Carrión, Callao; durante 7 meses (junio-diciembre, 2005). Para la identificación del S.
lugdunensis, se realizó el factor de afinidad para el fibrinógeno, la coagulasa libre en tubo, la
acidificación del manitol, así como las características del cultivo que presenta el S. lugdunensis.
Como prueba de tamizaje se utilizó la descarboxilación de la ornitina, y como prueba
confirmatoria se utilizaron carbohidratos de trehalosa y manosa. El antibiograma se realizó
mediante el Método de Difusión en Agar (Bauer y Kirby), según las normas de la CLSI-NCCLS
del año 2005. Se revisaron las historias clínicas de los pacientes con aislamiento de S.
lugdunensis.
RESULTADOS: Se obtuvieron 6 aislamientos de S. lugdunensis de las siguientes muestras:
hemocultivo (3), abscesos (2) y herida quirúrgica (1). En 5 casos el cultivo fue puro y en un caso
mixto. La localización de las lesiones fue: cuero cabelludo (1), pie (1) y región lumbosacra (1).
Tres de los seis aislamientos (50%) presentaban factor de afinidad para el fibrinógeno. Dos de
los seis aislamientos (33.3%) fueron sensibles a la penicilina. En 4 casos las cepas eran
productoras de betalactamasas (66.6%) y en 2 casos resistente a la meticilina (33.3%).
CONCLUSIONES: La frecuencia del S. lugdunensis en nuestro hospital es del 5% de todas
las cepas de estafilococos coagulasa negativo aislado en el laboratorio. Existe una elevada
resistencia a los antimicrobianos en comparación a estudios realizados en Europa y EEUU. Este
estudio establece un sistema de detección que sea factible de realizar por otros laboratorios de
microbiología.
