To Baptize Children 7 years of age and older – please contact Dr. Carmine in our Faith Formation Office at
(239) 574-2411. Para bautizar a niños de 7 años y mayores: pógase en contacto con el Dr. Carmine en

For the Baptism of a Child at least one parent must be Catholic. Para el bautismo de los niños, por lo menos,
uno de los Padres debe ser Católico.
Please choose Godparents who receive the Sacraments and will be a good example of living a Christian life.
One Godparent, male or female, is required, but there may be two, one of each sex, no less than 16 years of
age, and cannot be the mother or father of the child to be baptized. If the Godparents (or Proxies) are not
present, the Baptism cannot take place. Por favor, elija Padrinos que puedan recibir los sacramentos y que
sean un buen ejemplo de vida Cristiana. Un solo Padrino o Madrina es suficiente; pero si hay dos, deben ser
uno de cada sexo; por lo menos de dieciseis años de edad y no puede ser el Padre o la Madre del niño a
bautizar. Si el Padrino/Madrina o los delegados no están presentes durante el Bautismo, el Bautismo no
ocurrirá.
When all of these documents are received in our office, a baptism date will be set with you.
Cuando tengamos todos estos documentos en nuestra oficina, entonces elegimos conjuntamente con
usted la fecha del Bautismo.
1. Completed baptism registration form singed by both parents.
Formulario de registración de bautismo completamente lleno y firmado por ambos padres.
2. The Child’s birth certificate.
Copia de certificado de nacimiento del niño
3. One Godparent must be a Confirmed Catholic and present a Confirmation Certificate OR a letter from
the parish where he/she is registered, stating he/she is eligible to be a Godparent.
Uno de los Padrinos debe ser un Católico que haya hecho la Confirmación y debe presentar un
certificado de Confirmación O una carta de la parroquia donde está registrado, diciendo que es elegible
para ser Padrino.
4. The second Godparent can be a Confirmed or a Baptized Catholic or Christian and present a
Confirmation or Baptism certificate OR a letter from the parish stating that he/she is eligible to be a
Godparent. El otro padrino puede haber sido confirmado o bautizado en la Iglesia Católica o en otra
iglesia Cristiana y debe presentar un certificado de Confirmación o de Bautismo O una carta de la
parroquia donde está registrado, diciendo que es elegible para ser Padrino.
5. Both parents and both godparents are required to take a Baptism Class within two years of this
baptism. If the class is taken at another parish, a letter of attendance must be presented. Classes at St.
Andrew Church are given once a month. Please call to register for the English or the Spanish Class.
Se require que los padres y los padrinos asistan a la Clase Pre-Bautismal o que hayan asistido a estas
clases en los últimos do años. Si asisten a esta clase en otra parroquia, deben traer una carta de
certificacion. Estas clases en la Parroquia de San Andrés se realizan una vez al mes. Favor de llamar
para registrarse en las clases en Inglés o en Español.
