Parental Statement of Interest for Participation in the
Dual Language Program
If your child is attending pre-school, kindergarten or first grade next year in Maywood-Melrose
Park-Broadview School District 89 during the school year 2014-2015 and you are interested in the
dual language program, please complete this form and return to the District Administration Building
at 906 Walton Street, Melrose Park, IL 60160. You may also return it to a District 89 school near
you. You will be informed of registration procedures.
In the dual language program your child will learn reading, writing, math, science, social studies,
and other areas in two languages-English and Spanish. The goal of the program is high levels of
academic achievement in both languages.

Si su hijo/a va a asistir al pre-escolar, kindergarten o primer grado el año próximo en el Distrito
Escolar 89 de Maywood-Melrose Park-Broadview durante el año escolar 2014-2015 y usted está
interesado en el programa de lenguaje dual, por favor complete este formulario y regréselo al
Edificio de Administración del Distrito ubicado en el 906 Walton Street, Melrose Park, IL 60160.
También lo puede regresar a una de las escuelas del Distrito 89 más cercana a usted. Se le notificará
de la inscripción en el programa.
En el programa de lenguaje dual su niño/a aprenderá a leer, escribir, matemáticas, ciencias, estudios
sociales y otras áreas en dos idiomas-inglés y español. La meta del programa es alto nivel de logro
académico en ambos idiomas.
